Time is money, so little things may be cost prohibitive, but most printing companies are now dealing with high quality .jpg,  .tiff,  .eps, .gif, & .bmp files. I have the ability to save files in dozens of high quality formats and I have high speed Internet access, and I can e-mail the approved (by you) image to whatever printing company you prefer. Several companies have "upload pages" set up for this purpose, and most are specific about sizes, dpi, and pixels that may not be understood by most end-users.  The price of doing custom images can be off-set by the savings you receive by cutting down standard set-up fees.

A typical project takes around 30 mins. @ 
^$75.00 ^hr.
and depending on how the media is to be transmitted that will be it. If several long distance fax's are necessary that "cost" will be passed along to the customer. As stated before, "time = money."
